Online Library Card Registration

Once this form is submitted, it will be processed by LCPL staff and a physical card mailed to the address provided in the form. You MUST have this card with you to check out items at the library.

This is your personal library card, and is only able to be used by you.

***If card form is for a minor, use THEIR identifying information but YOUR contact information.***

Loan Periods

A maximum of twenty (20) total items may be checked out at one time to each patron. Books are loaned for fourteen (14) days with one (1) renewal.

***New adult patrons cannot check out DVDs or video games for the first 30 days.***

A maximum of four (4) DVDs are loaned to adult patrons only for two (2) days with no renewals. Magazines are loaned for seven (7) days with one (1) renewal. Video games are loaned for five (5) days to adult patrons only with no renewals. Reference materials are not available for loan, but may be viewed or copied in the library.

*** If patrons have overdue items on their library card computer access within the library will be denied. Patrons must either return the items or pay the replacement cost for computer access to be restored. The same policy applies for checking out items on Overdrive.

DVD Fine: $2/day
Video Game Fine: $5/day
Books and magazines are fine free, but we ask for a non-perishable food donation to the local food bank. Items may be left with staff at the desk.

I assume financial responsibility of materials borrowed by my child on his/her card. I understand that this card enables my child, under the age of 18, to borrow any materials, excluding movies and video games, from the library collection. I realize the responsibility of guiding my child in selection and use of library materials belongs to me and is not the responsibility of the Lawrence County Public Library.
