The Lawrence County Public Library is a Passport Acceptance Facility.

Routine Service: 6-8 weeks
Expedited Service: 2-3 weeks
Processing times only include the time your application is at one of the passport agencies or centers and do not include mailing times, which may vary across the country.
You must apply in person if ANY of the following are true:
- You have never been issued a U.S. passport
- You were under age 16 when your previous passport was issued
- Your most recent U.S. passport was issued more than 15 years ago
- Your most recent U.S. passport was lost or stolen
- Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change
HOW TO APPLY
Step 1: Complete a Form DS-11
- Fill out a Form DS-11 Online and Print It
- Print a Form DS-11 and complete it by hand
- Complete Form DS-11 at the library
Note: If printing a form, print single sided and bring all pages (including instructions) with you to your appointment. If filling the form out by hand, print legibly and use ONLY black ink. DO NOT sign the DS-11 form until the Passport Agent instructs you to do so.
Step 2:
Note: Copies of ID MUST contain the front and back of the ID on one page. The library has a copier and can assist you with copying your ID.
- Proof of citizenship (One of the following AND one black/white or color copy)
- Original birth certificate or a certified copy (no hospital certificates or photocopies)
- Previous U.S. Passport or Passport Card
- Naturalization certificate
- Certificate of Citizenship
- Consular Report of Birth Abroad
- Proof of identity (If applicable, bring along one black/white or color copy of the secondary ID)
- Valid Kentucky or West Virginia driver’s license
- If your ID was issued less than a year ago, you MUST bring a Department of State Approved Secondary Identification (Previously issued ID even if it is expired is acceptable)
- Passport Photo
- A 2 x 2 inch color photograph taken in the last 6 months is required. The picture should be of the full face with a plain white or off-white background, unfiltered, and no glasses. Have someone else take your photo (no selfies). Visit travel.state.gov’s “Passport Photos” page for full details. Walgreens or Wal-Mart offer photo printing.
- The Lawrence County Public Library can not take your photo for the application.
- Fees (two separate fees are required for a passport application)
- Application fee: payable to the U.S. Department of State by check or money order ONLY. Cash or debit/credit card cannot be used. See below for a listing of fees.
- Acceptance fee: A separate payment of $35 per application is required for any passport acceptance facility. Cash, check, or debit/credit card can be used.
Step 3: Schedule an Appointment (Service is by Appointment ONLY)
Appointments are able to be scheduled Monday through Friday, 10AM-3PM, unless we are closed for a holiday/in-service, with at least 24 hours advance notice. The Lawrence County Public Library hours and closings are available here. Also check the Library’s Facebook for any special closings.

In a hurry to get your passport?
If you are in need of a passport quickly (i.e. life or death emergency, traveling within two weeks), you can expedite your application. Please visit travel.state.gov’s “Get My Passport Fast” page prior to calling the library.
Applicants under 18 years of age
Applicants (Age 16 & 17)
- For applicants age 16 and 17, parental awareness must be shown. Parental awareness an be shown by a parent appearing with the applicant in person or a signed statement consenting to the issuance of a passport from one of the applicants parents or legal guardians along with a photocopy of their ID
- A Form DS-3053 (Statement of Consent: Issuance of a U.S. Passport to a Child) can also be printed and signed in front of a notary by the parent or legal guardian that is unable to be at the appointment.

Minor Applicants (Under Age 16)
- Both parents/guardians must authorize the issuance of the child’s passport

Renewals

Renewals CANNOT be handled by the Library. If you have a passport that was issued less than 15 years ago and you were at least 16 years old when it was issued, a self-service renewal by mail is likely the correct option. Visit travel.state.gov’s “Passport Renewal” page to confirm that renewal by mail is appropriate.
Note: If your last passport was issued when you were under 16, you will need to fill out a new DS-11 form. The Library accepts appointments for this form.
Passport Photos
The Lawrence County Public Library is unable to take your photo. You must bring your own passport sized photo (2 x 2 inch) as part of your application. Some Walgreens and Wal-Mart offer passport photos–they must specifically be passport photos.
Fees
Two fees are required for a new passport–one is a check or money order made payable to the U.S. Department of State, and one is to the Lawrence County Public Library for executing the application. The fee for the Library can be check, money order, cash, or credit/debit card.
